Nomination of Beneficiary

You may wish to nominate someone to receive any Death Grant payable in the event of your death. Please read the notes carefully before proceeding. You will need to print, sign and return the form if you wish to proceed. Please note that the documents are different depending upon when you ceased contributing to the Pension Scheme. Refer to the section that applies to you.
